Application Form: Fill out the Public Assistance application form.

Submission: Take the completed form to the nearest Human Services Office.
Local Board Review: The Local Board of Guardians in your district meets once a month to review applications. They will either approve or reject your application.
Approval Process
If your application is approved, you will receive your assistance book the following month.
The designated officer is responsible for ensuring you receive your book. They must submit the approved application to the Department for printing and to the Board for consideration in a timely manner.
Medical Assistance
If you are applying for assistance due to a medical reason, ensure that your doctor completes the medical reporting section of the application form.
Appeal Process
If your application is rejected, you can appeal the decision by requesting the officer to send your application for review to the Poor Law Commission.
Important Note
Public Assistance is temporary and is typically granted for a maximum of 2 years.
